Investigation Summary
At approximately 8:38 a.m. on September 17, 2009, an employee was moving a palette of merchandise with a Crown walker stacker type Model Number UL853E and did not have his left hand inside the safeguard portion of the control handle. As the employee turned the walker stacker in a clockwise direction, his left index finger got smashed between the vertical beam of the storage racks and outside portion of the control handle. A manager transported the employee to Pioneers Memorial Hospital in Brawley for treatment. The employee's left index finger was too damaged to reattach it and it had to be amputated. The employee was not hospitalized.
