Investigation Summary
At approximately 4:00 p.m. on April 23, 2007, Employee #1, a carpenter's apprentice, was working with a RYOBI 10-in. table saw to trim a 1.5-ft. by 4.5-in. piece of laminate wood flooring. The piece of flooring bound up, kicked back, and smashed his left hand, lacerating two fingers. The supervisor drove Employee #1 to the nearest medical center for emergency medical care. After evaluation, he was transferred to the regional medical center, where he was admitted for surgery, and hospitalized for two days.
