Investigation Summary
On October 11, 2007, an employee, a maintenance worker, was asked to adjust the ceiling louver vents of the air conditioning system in the lobby area. The employee set up an 8 ft. fiberglass ladder directly under the vent and proceeded to climb up the ladder to the fifth or sixth rung. The employee was adjusting the vent when he lost his balance and felt the ladder start to sway to the left and as he tried to hold on to the vent, the ladder fell to the floor and then he fell approximately 5 to 6 feet to the floor. After the accident, emergency medical services were summoned, and the employee was stabilized then transported to Glendale Memorial Hospital were he remained for approximately 4 days. Then, he was transferred to Cedar-Sinai Hospital for further treatment, were remained for approximately 7 days. The employee's injuries included fractures to the left hip and pelvic area.
