Investigation Summary
On June 27, 2005, a part-time laborer of Southern California Moving and Storage Co. was working in%09 a storage yard in Temecula, California. The employee was assisting a forklift operator who was removing a large, empty storage crate from a bobtail truck. The crate, held by the forks, tipped towards the employee who was pushed backward and fell to the ground. He did not appear to be injured and declined management's offer for medical care. Two days later the employee complained of a headache and was evaluated and treated at the Inland Urgent Care Clinic, in Wildomar, California. He was released and on June 30, he passed out. Paramedics took him to the Long Beach Memorial Medical Center where he was hospitalized until July 18.
