Investigation Summary
On January 31, 2006, Employee #1 and a coworker were standing on the seventh rung of two separate stepladders placed under each end of an overhead fluorescent light fixture. Employee #1 was dismantling one end of the fixture by loosening and sliding the clamp that held the fixture to a beam. When the clamp unexpectedly slipped off the beam, Employee #1 caught the light fixture, but lost his balance. Employee #1 then fell 7 ft to the ground. He was hospitalized at Highland hospital for a fractured L1 vertebra.
