Summary Nr: 200822542Event: 05/14/2004Employee Killed When Pinned Between Lifts
At approximately 9:00 a.m. on May 14, 2004, Employee #1 was backing a boom-lift (JLG Ultra-boom Extensible Boom Aerial Lift) into a bay and came in contact with a second boom-lift. Employee #1 was pinned between the two pieces of equipment. Employee #1 was found slumped over the controls with the other boom lift on top of him. Employee #1 was working alone. Employee #1 had not been trained in aerial lift safety. Employee #1 suffered a crushed head and was killed.
