Investigation Summary
On January 14, 2005, Employee #1 and a coworker were putting together a shelving rack. Employee #1 was holding one end of a horizontal member and the coworker was holding the other end. They were trying to attach the horizontal member to the vertical uprights. When they learned that the fasteners did not match the pieces they were working with, Employee #1 asked his coworker to call the supervisor. As the coworker left, he let his end of the horizontal member on the vertical upright, unfastened. The unsupported end dropped and caused the other end to turn over, pinching Employee #1's left middle finger between the horizontal member and the upright. Employee #1 received a partial amputated middle finger.
