Investigation Summary
At approximately 5:15 a.m. on December 5, 2009, Employee #1 was pulling an electric pallet jack backwards, when he walked into a forklift. The forks had been left in an elevated position and lacerated his lower right leg. He was transported to Hollywood Presbyterian Medical Center, where he underwent surgery and received approximately 50 sutures in his leg. After receiving the medical treatment, he was hospitalized and then released two days later.
