On October 16, 2009, Employee #1 was told to paint the inside of the building using a 6 ft ladder on top of a metal scaffold approximately 6 to 7 ft high. The ladder pushed the scaffold away, and he fell to a hard floor. Employee #1 sustained a fractured left arm and was hospitalized.
