Summary Nr: 142231.015 Event: 12/03/2021Employee With Covid-19 Infection Dies
On December 3, 2021, an employee who had been working as a full-time school teac her at at an education center teaching onsite classes died from COVID-19 infecti on. The employee began to experience COVID-19 related symptoms on November 5, 20 21, and tested positive for COVID-19 on November 12, 2021. On November 15, 2021 , the employee was hospitalized. The employer stated there was a COVID-19 outbre ak at the school which caused the school to close and conduct classes virtually from November 11, 2021 until November 29, 2021. The date, time and other details leading up to exposure are not indicated in the narrative. Information regardin g engineering, administrative and personal protective equipment controls for COV ID-19 are also not included in the narrative.
