At 8:00 a.m. on December 6, 2021, Employee #1 who had worked as a customer repre sentative died from COVID-19 infection. Employee #1 last worked on October 28, 2 021, and tested positive on October 29, 2021. Employee #2 had worked as an oper ator in the manufacturing department and also died from COVID-19 infection, alth ough the date is not indicated in the narrative. Employee #2 last worked on Nove mber 3, 2021. The employer did not have an effective COVID-19 Prevention Program in place.
