Accident Investigation Summary
Summary Nr: 140937.015 Event: 10/29/2021Employee With Covid-19 Infection Dies
An employee was transporting residents at a skilled nursing facility. The employ ee worked alongside a coworker (Transportation Aide) driving residents to doctor appointments. The employee also conducted maintenance work throughout the facil ity. Several coworkers and residents had tested positive for COVID-19 at the ti me of the employee's positive test result. On October 13, 2021, the employee sta rted showing COVID-19 related symptoms, so he took a COVID-19 rapid test and the test was positive on the same day. The employee was hospitalized a few days la ter due to complications related to COVID-19. The employee remained in the hospi tal until he died at 2:40 p.m. on October 29, 2021. Information regarding engine ering, administrative and personal protective equipment controls for COVID-19 ar e not included in the narrative.
