Accident Investigation Summary
Summary Nr: 141452.015 Event: 09/17/2021Employee With Covid-19 Infection Dies
An employee performed installation and repair work. On Friday, August 6, 2021, the employee and a coworker made a delivery to a customer. The employee and cowo rker both both began to feel ill and exhibited COVID-19 related symptoms over th e weekend. On August 8, 2021, both the employee and his coworker went to the hos pital. The coworker was provided a steroid shot and sent home the same day and t he employee was admitted for observation where he stayed until August 10, 2021. On August 10, 2021, the employee was released from the hospital and allowed to r eturn home to quarantine. On August 16, 2021, the employee returned to the hospi tal due to the severity of his symptoms increasing. The employee was admitted an d remained at the hospital until his death at 5:00 p.m. on September 17, 2021. I nformation regarding engineering, administrative and personal protective equipme nt controls for COVID-19 are not included in the narrative.
