| An employee was working as a licensed practical nurse in a physician's office. S he had received a negative result on a COVID-19 rapid test, but two days later, she received a positive result from a PCR test. The following day, she became sh ort of breath. She went to the hospital. She was admitted for treatment of her s hortness of breath and dehydration, and she underwent surgery for an infection s he was found to have in one of her wrists or hands. She died three days later, a t 4:44 p.m., on September 13, 2021, due to COVID-19. She had contracted COVID-19 from an unknown source. | ||||||
