Accident Investigation Summary
Summary Nr: 138411.015 Event: 08/17/2021Employee Is Killed When Crushed In Skid-Steer
At 3:30 p.m. on August 17, 2021, an employee was standing on the bucket attachme nt of a skid-steer tractor facing the operator compartment and instructing a cow orker on how to operate the skid-steer. The employee stepped on the left floor p edal of the tractor to demonstrate how raise the bucket. The bucket and lifting arms raised up quickly while the employee was still standing on the bucket. The employee's torso was caught between a cross-member on the lifting arm and the to p of the tractor cab. The employee suffered crushing injuries to his chest and n eck and died.
