Accident Investigation Summary
Summary Nr: 135079.015 Event: 04/08/2021Employee With Covid-19 Infection Dies
An employee was working at a department store. She was working as a style consul tant in the children's clothing department. Among the employee's duties, she loa ded clothing among merchandise arriving at the store on semi-trucks onto U-Boat carts and then placed the merchandise at the sales floor. The employee's last da y of work was March 29, 2021. She came down with symptoms beginning the same day at an unknown time. On March 31, 2021, she was tested for COVID-19. She receive d the positive results on April 5, 2021. On April 8, 2021, she was hospitalized. That same day, at 7:52 p.m., she died. At least eleven other employees at the w ork site tested positive within a two-week period. The employer made a late repo rt to the Cal/OSHA Fremont District office on April 20, 2021. The employer state d that a family member contacted them on April 17, 2021, to inform them of the f atality. After investigating if they had to report the fatality, they did so on April 20, 2021.
