Summary Nr: 134286.015Event: 03/15/2021Employee Is Killed While Performing Maintenance On Ship
At 8:00 a.m. on March 15, 2021, Employee #1 was in the process of removing, clea ning, preparing the surface, and replacing the blow-in door gasket seals on the USS McFaul. The door was tagged out in the open position however, in this posit ion the pneumatic pressure and electric power could not be secured. While perfor ming the work the door closed on the employee crushing her. She died from the re sulting traumatic asphyxiation and cardiac arrest.
