Accident Investigation Summary
Summary Nr: 133051.015 Event: 01/07/2021Employee With Covid-19 Infection Dies
On January 7, 2021, Employee #1 who had been working as a Licensed Practical Nur se (LPN) providing direct patient care at a rehabilitation and health care cente r started exhibiting COVID-19 related symptoms. Employee #1 was confirmed to hav e been exposed to COVID-19. Employee #1 was hospitalized 5 days later and died d ue to COVID-19 related complications on February 3, 2021. The employer provided N95 respirators and established several administrative controls (social distanci ng, employee and patient testing) in an attempt to control the spread of COVID-1 9. Employee #1 worked a double shift with three residents who had tested positiv e for COVID-19 following her shifts. COVID-19 positive residents were isolated i n their private rooms and staff conducting work inside the rooms were provided a nd required to wear Personal Protective Equipment (PPE) such as gloves, gowns, e ye protection, and N95 respirators. Signage was provided to instruct employees o f positive COVID-19 residents and the required PPE. Staff were required social d istance during shifts including breaks and while at workstations. However, the e mployer failed to fit test employees or obtain respirator medical clearance from a physician or other licensed health care professional (PLHCP).
