At approximately 11:45 a.m. on December 8, 2020, an employee was returning to th e work area on the production side of the shelving. The employee was a pedestria n at the time. The employee was struck by a powered industrial vehicle in the vi cinity of the aisle and shelving area. The employee was struck and pinned betwee n the vehicle and the shelving unit. The employee suffered internal crushing inj uries and died from these injuries.
