| At 10:00 a.m. on October 23, 2020, an employee worked as a Operator Laborer. His job included maintenance or equipment and cleaning. The employee tested positiv e for COVID-19 on 9/16/20 after not feeling well beginning on 9/12/20. While wor king, the employee wore face covering and maintained social distancing. There we re no close contacts at work who were known to be COVID-19 positive. There were no family members who were COVID-19 positive as they had been tested with negati ve results. The employer did an investigation to attempt to find the source, inc luding asking about travel, visits to a gym, theater, casino etc. within 14 days of onset of illnesses to which all answers were negative. The employee died fr om COVID-19. | ||||||
