Summary Nr: 127885.015Event: 07/17/2020One Employee Is Killed And Other Is Injured When Struck By T
At 8:55 p.m. on July 17, 2020, Employee #1 and #2 were attempting to set chocks and bracing blocks. They were attempting to chock and add bracing to stack the metal utility arm when one arm rolled on the forklift causing the other utility arm on the forklift to roll and strike both employees on opposite ends of the utility arm. Employee 1 received multi-system traumatic head chest and abdomen injuries and was killed. Employee #2 sustained pelvic and back injuries, resulting in inpatient hospitalization.
