Accident Investigation Summary
Summary Nr: 126623.015 Event: 06/01/2020Employee With Covid-19 Infection Dies
At approximately 8:00 a.m. on June 1, 2020, Employee #1 died from COVID-19 and was first diagnosed with COVID-19 on May 30, 2020. The employee worked at a re gional general and surgical hospital. The employee worked as an ultrasound tech nician and worked with COVID-19 positive patients prior to diagnosis of COVID-19 . According to the employee's family, the employee traveled to visit family in S an Diego where the family rented a house, with approximately 19 extended family members. They rented a vacation house in San Diego and did not wear facial cover ings or practice social distancing between family members. All other family memb ers tested negative for COVID-19 as they were tested after the employee was diag nosed with COVID-19 on May 30, 2020. It cannot be determined where or how the em ployee was exposed to COVID-19 or whether it was work-related or not.
