This investigation was associated with inspection number 1473032, for the tempor ary employment agency, and 1473037, for the secondary employer. At 1:15 p.m. on April 22, 2020, an employee was working for a temporary staffing or employment a gency. That was his primary employer. He had been sent to work at a firm that en gaged in specialized long-distance freight trucking. That was his secondary empl oyer. He finished lunch and walked back up to an excavator, unaccompanied. A cow orker found him face down on the ground. His death was thought to be due to a pr e-existing medical condition, likely dysrhythmia. No causal link to any work act ivity was established.
