At 11:20 p.m. on December 18, 2019, an employee climbed up an eight foot step la dder and pulling a 20 lb box with both hands to place it onto the mezzanine when it started to slip and fall. The employee try to grab the box and lost her bal ance, falling backwards from the ladder and struck her head against the floor, f racturing her skull and was bleeding from her brain. The employee was killed.
