At 8:16 p.m. on December 6, 2019, a 58-year-old male employee was working as a laser operator for a manufacturer of high end household refrigerators and home freezers. A coworker there was also working as a laser operator. At approximately 8:00 p.m., the coworker-laser operator returned from his break. He was resuming production. At 8:03 p.m., the worker-laser operator noticed that the laser machine he was assigned to wasn't working. At 8:05 p.m., he manually reset the material handling light curtain at the laser pedestal. At approximately 8:05 p.m., the coworker-laser operator heard the employee yelling for help. The coworker-laser operator and a second coworker walked around the laser several times. They could not locate the employee. At 8:11 p.m., the coworker-laser operator found the employee. He had entered the laser machine's material handling area. He was lying on the laser machine's material handling index table. An overhead lift was resting on top of him and pushing down. The employee suffered multiple fatal crushing injuries. He was killed. It was not known why the employee was in the material handling area at the time of the incident. | ||||||
