Accident Investigation Summary
Summary Nr: 122291.015Event: 12/06/2019Employee Is Crushed And Killed By Overhead Lift
At 8:16 p.m. on December 6, 2019, a 58-year-old male employee was working as a l aser operator for a manufacturer of high end household refrigerators and home fr eezers. A coworker there was also working as a laser operator. At approximately 8:00 p.m., the coworker-laser operator returned from his break. He was resuming production. At 8:03 p.m., the worker-laser operator noticed that the laser machi ne he was assigned to wasn't working. At 8:05 p.m., he manually reset the materi al handling light curtain at the laser pedestal. At approximately 8:05 p.m., the coworker-laser operator heard the employee yelling for help. The coworker-laser operator and a second coworker walked around the laser several times. They coul d not locate the employee. At 8:11 p.m., the coworker-laser operator found the e mployee. He had entered the laser machine's material handling area. He was lying on the laser machine's material handling index table. An overhead lift was rest ing on top of him and pushing down. The employee suffered multiple fatal crushin g injuries. He was killed. It was not known why the employee was in the material handling area at the time of the incident.
