At 7:45 a.m. on November 11, 2019, Employee #1 was cleaning debris from materials, while performing temporary work (buildings and facilities), during a residential construction project. As Employee #1 cleaned these materials, he then inadvertently fell through an opening, landing approximately 25 feet below to the basement from the second floor. Employee #1 suffered blunt force trauma as a result of his injuries and was killed. | ||||||
