Accident Investigation Summary
Summary Nr: 118174.015Event: 07/22/2019Employee Is Killed In Fall When Guardrail Fails
At 9:45 a.m. on July 22, 2019, Employee #1 and a coworker were installing insulation at a new four-story residential structure. The coworker was pulling insulation hose from their box truck, and Employee #1 was on the structure's fourth floor and pulling up the hose to an unfinished balcony. Employee #1 leaned against a guardrail set across an opening where a window was to be installed. The guardrail came loose, and Employee #1 fell through the opening, a fall height of approximately 21 feet, and landed on his back on the second floor balcony. He was killed. The subsequent investigation determined that the guardrails set into the unfinished balcony were not construction grade lumber, were not properly secured during installation, and were less wide than the balcony's window opening.
