Summary Nr: 117461.015Event: 06/24/2019Employee Is Killed When Forklift Rolls Over
At 2:00 p.m. on June 24, 2019, an employee was operating a Toyota Forklift Truck, Model 8FGU20, while loading a tractor trailer. The employee turned the corner and lost control of the forklift before it rolled over at the end of the ramp. The employee sustained blunt force trauma injuries to his head, neck, and trunk and was killed.
