On June 7, 2019, an employee and coworker were moving a welding jig that weighed approximately 1,000 lbs. The coworker raised the jib approximately six inches f rom the ground while the employee leaned his body into the jig. The jig wobbled and slide off of the forks, striking the employee. The employee was crushed an d killed.
