Accident Investigation Summary
Summary Nr: 120123.015 Event: 03/30/2019Employee Falls From Order Picker, Suffers Multiple Injuries
At 7:00 a.m. on March 30, 2019, an employee was operating an order picker workin g at a height of approximately 6 feet in order to retrieve a case of olives. The employee stepped onto an empty pallet on the second level of the rack when it g ave way. He fell to the first level rack below landing on his side on a pallet a nd boxes of product. He fractured lumbar and thoracic vertebrae, fractured his r ibs, suffered an fibular avulsion fracture at the left knee and a concussion and was hospitalized. He was not wearing the provided personal fall protection at t he time of the accident.
