At 9:00 p.m. on April 4, 2019, three employees were mating a base skirt section weighing 21, 559 lbs to the main part of a pressure vessel. Employee #1 and Employee #3 were taking measurements of the skirt while it was elevated by slings approximately 4 feet above the floor when the gravity center shifted and the skirt fell from the slings, striking Employee #1 and Employee #3. Employee #2 was pushed away by Employee #1 and fell to the ground. Employee #1 had his left arm and leg severed as well as sustaining blunt force trauma to his torso and died enroute to the hospital. Employee #3 was pronounced dead at the scene. Employee #2 sustained minor scrape and bruises; however, he did not need hospitalization. | ||||||
