Accident Investigation Summary
Summary Nr: 113921.015Event: 02/11/2019Employee Suffers Trauma After Being Pinned By Side Loader
At approximately 2:05 p.m. on February 11, 2019, an employee was rebooting a sca nning gun due to connectivity issues to scan materials prior to the incident. Th e employee and coworker #1 was working in aisle 11, but both employees overlook ed to place the safety cones at the entry points. Safety cones at the entry of t he aisle alert others that the aisle is occupied which means do not enter. Cowo rker #2 was operating a side-loader picking orders and went down aisle 11 and wa s looking up searching for his pick. Then, coworker #2 struck the employee as th e employee pushed coworker #1 out of the way. The employee was pinned between t he rack and the side-loader. Coworkers called 911, and upon arrival, EMTs asses sed the injuries of the employee and transported him to the hospital. The emplo yee sustained injuries in the accident: broken ribs, fractured pelvis, and a bru ised lung.
