Accident Investigation Summary
Summary Nr: 116947.015 Event: 01/15/2019Employee Amputates Pinky While Working On Harvester
At 8:20 a.m. on January 15, 2019, an employee and his coworker were in a broccol i field working on a fabricated broccoli harvester assembling and loading boxes of broccoli heads. A chain system was used to pull a conveyor to move packed box es from the packing stations to the area in the harvester where they were loaded onto pallets. There were approximately 3 to 4 boxes full of product on the conv eyor when the conveyor came to a stop due to the chain coming off its track. The employee informed his coworker that the conveyor had come to a stop and was not moving. The coworker instructed the employee to tum off the motor and then plac e the chain back in place. The employee accidentally accelerated the conveyor in stead of stopping it. When he lifted up the chain with his left hand to unjam it , the conveyor reengaged its movement, catching his left pinky finger between th e conveyor and a safety guard. The employee sustained a partial amputation of hi s left pinky finger. The employee was hospitalized.
