At 6:15 p.m. on January 8, 2019, Employee #1 was operating a Toyota Electric For klift Truck, Model Number 8BNCU20 and Serial Number 8BNCU20-56687. A roll of shr ink wrap that was sitting on the forklift shifted, and struck the joy stick, sen ding the truck into the metal racking. The employee's left leg was pinned and fr actured when caught between the forklift truck and metal racking. Employee #1 wa s hospitalized for his injury.
