Summary Nr: 112572.015Event: 01/10/2019Employee Is Crushed And Killed By Falling Forklift
At 2:00 p.m. on January10, 2019, an employee was servicing/ changing the oil on a Caterpillar forklift Model Number: GC40KS, Serial Number: AT87A00203. Coworke rs lifted the forklift with a "sky track" so that the employee could access the oil filter. The forklift slipped from the "sky track" and fell inclined on the t wo wheels (on one side). The employee was harnassed on the side of the forklift. The forklift fell completely on its side and crushes the employee. The employe e was killed from a skull fracture.
