Summary Nr: 112456.015Event: 01/04/2019Employee Is Pinned By A Conveyor And Killed
At 1:45 a.m. on January 4, 2019, an employee was loading packages inside a trail er and the extendible conveyor engaged and moved towards the employee. The emplo yee's back was turned towards the conveyor when the conveyor pinned the employee against the packages. The employee's coworkers were alerted by the maintenance monitoring system and "light" monitoring system at which time they found the emp loyee. The coworkers were unable to access the conveyor's controls because they were pinned between the employee's lower back and the boxes, so they proceeded t o 'dig out' the victim by removing the packages. Once freed, the employee's cowo rkers provided first aid until emergency medical personnel arrived and transport ed the victim to the hospital. The employee was pronounced dead as a result of t he injuries sustained in the fatal incident. Just prior to the incident, securit y video showed the employee controlled extendable conveyor slowly extending inwa rds into the trailer where the employee was located.
