Accident Investigation Summary
Summary Nr: 113655.015Event: 12/03/2018Employee Is Caught In Auger And Is Killed
At 10:00 a.m. on December 3, 2018, Employee #1 was operating a Diedrich D50 Drill Serial #423 to gather soil samples from a potential future construction site. The auger was brought out of the soil and while wearing leather gloves, Employee #1 used their hands to clean soil from the auger while it was still operating. Employee #1's hand became entangled in the auger and was drawn into/around the auger and against the machine. Employee #2, an assistant and not an operator, hit the e-stop and called 911. Employee #1 was pronounced deceased at the scene.
