Accident Investigation Summary
Summary Nr: 110465.015Event: 10/24/2018Employee Is Killed When Struck By Counterweight
At 10:45 a.m. on October 24, 2018, an employee was using a smoke vacuum to remov e welding fumes as a coworker welded a piece of angle iron onto a main beam. An adjacent elevator traveled upwards as its counterweight traveled downward while the employee and the coworker were still working. The distance from the counter weight to where the employees were working was approximately 16-19 inches. The e mployee was struck in the head by the counterweight of adjacent elevator, result ing in a fatal head injury.
