Accident Investigation Summary
Summary Nr: 110122.015Event: 10/16/2018Employee Falls Through Ceiling And Is Killed
At 3:36 p.m. on October 16, 2018, an employee was walking around a storage area that was located on a enclosed mezzanine type deck. The employee opened a door a nd fell through the false ceiling on the other side of the door because the area behind the door was beyond the mezzanine deck and the area was actually just th e ceiling for the store room below. There were two sliding bolt latches on the d oor and the door was not hanging square. Some force was required to open and cl ose the door, to and from the closed position. The investigation revealed that t he momentum of the employee pushing the door open caused the employee to lose hi s balance and fall through the door opening and through the false ceiling. The employee suffered blunt force trauma to his head, resulting in his death. The em ployee's legs struck some card board boxes during the fall and his head struck t he concrete floor.
