At 8:30 a.m. on October 10, 2018, Employee #1 was working at a multiemployer con struction project, a multifamily residential structure. He was on the roof when he slipped and fell, a fall height of 12 feet. The employee was transported to the hospital, admitted, and treated for a fractured arm.
