Summary Nr: 109849.015 Event: 10/02/2018Employee'S Fingers Are Crushed By Rollers, Later Amputated
At 8:50 p.m. on October 2, 2018, Employee #1 was cleaning the in-running dough r ollers of a pasta dough machine with a rag. He did not lift the guard, which sto ps the rollers. The rag, followed by his two fingers of his left hand were pulle d into the in-running rollers and crushed. The two fingers had to be surgically amputated and the employee was hospitalized for treatment.

Summary Nr: 102952.015 Event: 02/05/2018Employee Falls From Top Of Wall And Suffers Back Injuries An
At 3:20 p.m. on February 5, 2018, an employee was on top of a 9 foot wooden fram ed wall in a building that was under construction and was nailing braces on the top portion of the wall to the plumb wall. The employee turned to nail the next brace, lost his balance, and fell from the wall to the floor, landing on top of his hammer. The employee was hospitalized with lower back pains and a sprained left hand.

At 1:30 p.m. on November 16, Employee #1 was sweeping the ground. A coworker, a forklift operator, was transporting a 3-yard trash bin on the forks, and was mo ving to exit the building. The coworker made a turn, and did not see Employee #1 , who was sweeping the ground next to the exit. Employee #1 was struck by the t rash bin and died 5 days later at the hospital.
