Summary Nr: 109286.015Event: 09/19/2018Employee Is Struck And Killed By Falling Machinery
At 10:30 a.m. on September 19, 2018, an employee was assisting with unloading la rge machinery from a flatbed truck. As a coworker was using a forklift to unload the machinery from the flatbed truck, the employee was signaling. During unladi ng, the employee inadvertently stepped between the load and the flatbed truck as it was being lowered. The machinery fell forward and struck the employee, who b ecame pinned to the ground. He suffered sever crushing injuries, resulting in hi s death.
