Summary Nr: 109117.015Event: 09/13/2018Employee Is Struck By Skid Steer Grappler Spike And Is Kille
At 12:00 p.m. on September 13, 2018, an employee was cleaning up the work site i n preparation of departing the site for the day. The employee, operating a Verme er S725 mini skid steer, had disconnected a bucket attachment from the loader an d attached a grappler attachment in order to pick up five sheets of 4 foot by 8 foot plywood that was used to prevent the tracks of the loader from tearing up t he resident's yard. The grappler's means of attachment to the skid steer was not properly secured. The grappler and plywood fell and one of the spikes on the g rappler fatally pierced the employee's head.
