Accident Investigation Summary
Summary Nr: 109101.015Event: 09/09/2018Employee Amputates Finger While Using Stamping Press
At 11:15 p.m. on September 9, 2018, an employee was finishing the first batch of blanks for a production run on the 801 stamping press. The employee noticed that the alignment of the blanks was off and placed his hands on the top of the stack to lift himself up. His coworker crossed the photo eye at the front of the piler with a wooden stick and caused the plier to clamp down, pinching the left middle finger and amputating it at the nail. The employee was transported by Emergency Medical Services to Riverside Methodist Hospital.
