Summary Nr: 109100.015Event: 09/12/2018Employee Is Struck And Killed By Several Falling Metal Doors
At 1:10 p.m. on September 12, 2018, an employee was offloading miscellaneous har dware supplies from a 40-foot shipping container. Stored at the front of the co ntainer were seven solid core doors (positioned vertically) and approximately fi fty boxes of various sizes stored next to the doors. During offloading of the b oxes, the seven solid core doors fell and crushed the employee's head and neck. Total weight of the seven solid core doors was 672 pounds. The employee was kill ed.
