Accident Investigation Summary
Summary Nr: 109133.015Event: 08/21/2018Employee Injures Leg In Fall While Using High Pressure Hose
At 10:45 a.m. on August 21, 2018, an employee was using a rodder hose to clean debris from the scrapper (left to right). He straddled the rodder hose and held it with both hands waist high and then moved the hose over his shoulder for a better angle. The back pressure caused the employee to lose his balance on and fall onto the street sustaining an injury to his left leg quadriceps tendon requiring an in-patient hospitalization. It was determined that the rodder hose had an "aftermarket" brass nozzle attachment, instead of the original manufacturer's water spray hand gun. Also, the employee of the rodder hose was not trained in the use of the equipment.
