Accident Investigation Summary
Summary Nr: 109047.015Event: 09/06/2018Employee Injures Head In Fall From Ladder
At 10:49 a.m. on September 9, 2018, Employee #1 was working in the back storage room of a souvenir and costume retailing location in Marina, CA. Employee #1 was using an 8-foot Werner fiberglass stepladder to reach a box containing metal shelving prongs on the upper level of the storage rack. Employee #1 was attempting to slide a box containing metal prongs closer to the left side of the shelf when the front left shelving corner hook tab disengaged from its upright post and the top shelf collapsed. Employee #1 fell backwards and hit the her back and head onto the polished concrete floor. A coworker, a sales associate was working nearby and ran out to the front of the store to ask another coworker to contact 911. The employee was hospitalized.
