At 9:45 a.m. on August 16, 2018, Employee #1 was using a SuLee Model FA-200 automatic cutter, cutting cloth for curtains. The employee placed his left hand down to hold the cloth in place, and pressed the cycle button with his right hand. The employee sustained an amputation injury to the left index finger, as well as causing injury to the middle and ringer fingers.
