At 2:30 p.m. on June 26, 2018, an employee was walking the wall of a bunker silo to remove tires and plastic to prepare the bunker for feeding cattle the following morning. The wall was approximately 11.4 feet tall and 1.3 feet wide. After accessing the wall by way of a metal ladder and walking about 23 feet of the wall, the employee fell and struck his head on the cement floor below. He suffered a fractured skull that resulted in his death. | ||||||
