At 2:45 p.m. on June 15, 2018, an employee was transitioning from a ladder to a roof while attempting to bring a coworker a caulk gun. The employee was not tied off to an anchorage point while walking on the roof when he slipped and fell approximately 15 feet to an asphalt surface. The employee sustained a fractured tailbone and wrists.
